Renin-Angiotensin System Inhibitors and Prevention of Diabetes

Abstract
Type 2 diabetes mellitus (T2DM) is a major health problem all over the world. To reduce the burden from T2DM, prevention of the disease should be considered as a key objective in the near future. A growing number of experimental and clinical studies have provided evidences that pharmacological blockade of the renin-angiotensin system (RAS) by either angiotensin-converting enzyme (ACE) inhibitors or angiotensin receptor blockers (ARBs) might reduce the incidence of new onset T2DM in high-risk subjects. The beneficial effects of RAS inhibition on the development of diabetes have been largely attributed to improvement in peripheral insulin sensitivity and glucose metabolism. However, the main mechanism of the protective effect of ACE inhibitors or ARBs remain to be determined. In addition, prospective, randomized controlled studies are still needed to further elucidate their exact roles in diabetes prevention.
